Why body composition matters on a GLP-1
GLP-1 medications such as semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) can produce large weight loss. The bathroom scale only shows the total. A DEXA scan splits that total into fat, lean mass and bone, so you can see how much of the change is fat.
The research is mixed. A 2024 review in Diabetes, Obesity and Metabolism found that across GLP-1 trials, lean-mass loss ranged from 40-60% of total weight lost in some studies to about 15% or less in others.[2] A 2025 meta-analysis of 38 publications (1,735 participants) found muscle-mass measures made up under 20% of total weight loss, both in people with type 2 diabetes and in people without diabetes, while most of the loss was fat.[1] The same paper notes reports of lean or fat-free mass losses as high as 40% of weight lost.[1]
Researchers who reviewed MRI studies concluded that the muscle changes seen so far look largely adaptive rather than harmful, with signs that muscle quality improves, while flagging older age and frailty as reasons for extra attention.[3][2] The AACE 2025 obesity algorithm recommends resistance training at least 2–3 days a week and protein of at least 1.2 g/kg of body weight per day for most people during weight loss.[5] Ask your prescriber or a registered dietitian what that means for you.
What DEXA measures, and its limits
A whole-body DEXA (DXA) scan reports fat mass, lean soft tissue and bone mineral for your arms, legs and trunk, and usually an estimate of visceral fat. It takes about 7–10 minutes lying on an open table. More on how DEXA works.
- Lean mass is not muscle. DEXA "lean" means everything that isn't fat or bone: muscle, but also organs, blood, water and the non-fat part of fat tissue. Fat-free mass is about 74% water, and adipose tissue itself is roughly 15–20% non-fat material (mostly water and protein), so losing a lot of fat automatically lowers "lean" mass even if no muscle is lost.[4] The 2025 meta-analysis makes the same point: DXA picks up lean tissue in every compartment, so true muscle loss is probably smaller than the lean-mass number.[1]
- Hydration and food move the number. In a study of active people, scans taken after normal daily activities or after eating breakfast showed higher lean-mass readings and more measurement error than rested, overnight-fasted morning scans.[6] BodySpec also tells clients to reschedule after illness, travel or during menstruation because fluid shifts affect lean mass.[8] If GI side effects or a bug have left you dehydrated, a scan that day may not be comparable.
- Small changes can be noise. Repeat scans only mean something when the change is bigger than the scanner's measurement error, which is why a standardized routine matters.[7][8]
- It isn't a strength test. DEXA measures mass, not how well your muscles work. Your own lifting numbers and how daily tasks feel are useful context.
How often to scan
No medical guideline sets a DEXA schedule for GLP-1 users. A practical pattern is:
- Baseline before you start, or as soon as you can after starting.
- Repeat every 3 to 6 months while you're losing weight. AACE suggests checking the early response to an obesity medication after about 3 months on the treatment dose.[5] BodySpec, a DEXA provider, suggests every 8–12 weeks during active change and every 4–6 months in maintenance, and warns that changes over 6–8 weeks are often within measurement error.[8]
- Maintenance: spread scans out once your weight is stable.
Your prescriber may want something different, especially if you're older or have bone or muscle concerns. A fitness body-composition DEXA is not a diagnostic hip/spine bone-density exam.
Tips for consistent scans
A standardized routine is the most practical way to keep measurement error down.[6][7]
- Same lab, same machine. Different scanners and software can give different numbers. Book repeat scans where you had your baseline.
- Same time of day. Morning is easiest to repeat.
- Same fasting and fluids. Ideally scan fasted, or keep food and drink the same before every scan. Avoid big swings in hydration.[6][8]
- Same pre-scan routine. Skip hard exercise beforehand (BodySpec says 24 hours) and wear similar minimal, metal-free clothing.[8]
- Reschedule if you're unwell, just back from travel, or noticeably dehydrated.[8]
- Look at the trend across several scans rather than reacting to one small change.
- Ask about packages. If you'll scan every few months, a multi-scan pack or membership can cost less per scan than one-off bookings.

An alternative or add-on: RMR testing
A resting metabolic rate (RMR) test measures how many calories you burn at rest from your breath, usually in 10–20 minutes. It doesn't measure muscle or fat, but some people use it with a dietitian to plan calories as their weight changes, or pick it as a cheaper single check when a DEXA scan isn't available nearby. Published RMR test prices have a US median of $125 across 69 prices. Compare RMR test prices by city.

FAQ
Do I need a DEXA scan if I'm taking Ozempic, Wegovy, Mounjaro or Zepbound?
No one needs one to take these medicines, and no medical guideline requires routine DEXA scans for GLP-1 users. The American Association of Clinical Endocrinology's 2025 obesity algorithm says body-composition tools such as DXA are valuable for monitoring fat-free mass and muscle changes during medication-driven weight loss, but that their clinical use is limited by interpretation, availability and cost. A scan is optional information you can bring to your prescriber.
How much of the weight lost on GLP-1 medication is muscle?
It varies by study. A 2024 review found lean-mass loss ranging from about 15% or less to 40-60% of total weight lost across trials. A 2025 meta-analysis of 38 publications (1,735 participants) found muscle-mass measures made up under 20% of total weight loss. Lean mass is not the same as muscle, so these figures likely overstate true muscle loss.
How often should I get a DEXA scan on a GLP-1?
There is no official schedule. A practical pattern is a baseline scan near the start, then a repeat every 3 to 6 months. AACE suggests checking the early response to an obesity medication after about 3 months on the treatment dose, and BodySpec, a DEXA provider, suggests rescanning every 8-12 weeks during active change and every 4-6 months in maintenance, warning that changes within 6-8 weeks are often within measurement error.
Why did my lean mass drop even though I'm lifting?
Part of it may not be muscle. DEXA lean mass includes water, organs and the non-fat part of fat tissue itself, which is roughly 15-20% water and protein, so losing a lot of fat lowers lean mass automatically. Eating, drinking and daily activity before a scan also shift DEXA lean-mass readings. Compare scans taken under the same conditions and look at the trend across several scans.
How much does a DEXA scan cost?
Across 110 published single-scan prices in 22 US metros (checked 2026-10-26), DEXA body-composition scans range from $59.95 to $325, with a median of $149. Memberships and multi-scan packages can lower the per-scan price if you plan to repeat.
Is a DEXA body-composition scan the same as a bone-density test?
No. A fitness body-composition DEXA scans your whole body for fat, lean and bone totals. A diagnostic bone-density exam (hip and spine) is a medical test ordered by a clinician, often billed to insurance, and is usually repeated no sooner than every 2 years unless your doctor needs it sooner.
